Position Title: FULL-TIME MEDICAL SOCIAL WORKER

DUTIES:
• Social work evaluation of patient/family psych/social and environmental needs
• Implements the social work treatment plan and documents consistent with insurance guidelines in their homes or facilities
• Assists the primary nurse with establishing and maintaining a treatment plan for each patient/family.
• Provides support to selected patients post discharge.
• Represents the philosophy and services of Hospice of Huntington, Inc.

POSITION QUALIFICATIONS:

• Bachelor’s Degree or Master’s Degree in Social Work from an accredited school of social work required
• LICSW, desired
• West Virginia and Ohio Social Work license is required.
• Valid driver's license along with an insured, reliable vehicle required.
• Minimum of one year experience in health care, with a preference in Hospice/Home Health Care.
• Evidence of clinical experience and skills pertinent to assessing and counseling individuals.
