We are seeking a Licensed Vocational Nurse to join our team of medical professionals. The successful candidate will be responsible for providing quality care to patients in hospice settings. The ideal candidate should have a valid LVN license, communication skills, and the ability to work independently and as part of a team. This is an excellent opportunity for an experienced LVN to make a difference in the lives of patients and their families.

Responsibilities:
Patient assesments
Inform patient on instructions for medication prescribed by a physician.
Take and asses vital signs, record patient information, and report changes to the team of physicians.
Provide emotional support to patients and their families.
Provide simple wound care if needed
Maintain patient records in accordance with established standards.
PRN vists if neccessary.
Medication reconciliation.
Checking overall well being of patient and any care they may require.
