Full-time Case Manager - Guide on Site

Waikiki Health is recruiting for a full time Case Manager. The Case Manager will provide services to young people ages 18-24 who are homeless and at-risk of homelessness. Identify eligible young people and assist them with secure housing, provide case management, teach independent living and social skills. Provide employment/vocational/education resources, and provide any other services needed to support the young person to successfully live in the community.

Duties include but are not limited to:

- Receive and process referrals, gather appropriate documentation and ensure documents are completed and signed.

- Provide appropriate assessments and service planning for young people and their families.

- Role modeling healthy behaviors and coaching young people build positive skills.

- Participate in team meetings, CES meetings, attend court hearings, and other team meetings pertaining to the youths’ services and advocate on youths’ behalf.

- Be responsive as appropriate to crisis situations; provide crisis management, coaching and support.

- Other duties as assigned

Qualifications:

- College degree required. Health-related degree preferred.

- Must have at least 1 year of experience in youth and young person’s services.

- Must be organized and able to complete all assigned tasks.

- Hawaii driver’s license with clean 5-year history loss from their car insurance.

- Ability to transport young adults and their children in personal vehicle.
